To make your appointment quick and seamless, we encourage all new patients to fill out our New Patient Paperwork online before your appointment. Upon scheduling your first appointment, you will be invited to securely complete all paperwork electronically (on your mobile device or desktop). Should you prefer to complete this paperwork on traditional paper forms, please contact our office and we can arrange for downloadable paperwork (PDF) to be emailed to you.

Don't see your insurance provider?

An Insurance-Friendly Office

Take the load off stressing about insurance. We accept the vast majority of PPO dental insurance plans and are happy to submit claims on your behalf for your convenience.

While we may not be in-network with every insurance plan, many PPO plans still provide valuable out-of-network benefits. In these cases, your insurance may reimburse a portion of your treatment costs just as they would with an in-network provider, though coverage levels and patient responsibility can vary depending on your specific plan. Our team is happy to help verify your benefits, provide treatment estimates, and answer any questions you may have so there are no surprises.
